3.10.3 
DataManagement 
A 
licensee or permit-holder shall document monthly AVO inspections. 
3.10.4 
Reporting 
A licensee or permit-holder shall submit an annual report not later than 90 
days of the preceding year stating level of compliance and record of any 
deviations, along with certification of the truth and accuracy of report signed 
by 
a competent person.
