Step 2:
Create your company profile/login to your existing company account
Step 3:
Complete the online application form
Step 4:
Make payment
Step 5:
Attach and upload all applicable documents
Step 6:
Submit your application

Provision of Information and Documents on MDOGISP
3.2.1
Application Procedures and Instructions
The following are to be ensured:
a. All applications shall be duly completed and submitted via the portal. Companies are
not required to submit hardcopy documents.
b. Company must ensure to provide correct and functional addresses (registered and
operational), telephone numbers and e-mail at the time of submission.
c. For Document Upload, the following applies.
i.
Any document to be uploaded shall be clearly scanned from original documents
or certified true copies.
ii. All required documents shall be uploaded to the portal in PDF format. PNG or JPEG
formats are accepted for single paged documents. iii. Documents to be uploaded
on the portal shall not exceed 4MB.
iv. Uploading of zipped documents are not allowed.
