4.2.5
Documentation and Reports
All service companies shall have documentation systems for easy retrieval of information. It shall
ensure as follows:
i.
Established systems for maintaining information files and processing paperwork for easy
retrieval.
ii.
Documented standard working documents for all process and work activities.
iii.
Detailed instructions, drawings, or specifications are used to carry out all activities.
iv.
The accuracy and quality of reports that is capable of providing guidance and expert
advice to the Operator/Licensee.

SANCTIONS
Non-Compliance With the Requirements of these Guidelines shall be deemed as violations to
relevant sections of the Petroleum Industry Act 2021 and Regulations pursuant to the Act. These
violations may result in applicable consequence management, including but not limited to, fines,
penalties, revocation of permit and/or blacklisting of service provider. These Guidelines is subject
to review without notice.
